Go to run and type
diskmgmt.msc and click ok
Right click on your USB >>> format
If that doesnt solve the issue
Try this
https://ccm.net/downloads/tools/4639-hp-usb-disk-storage-format-tool/
Still you face the problem ,REPLACE it
