Create a infopath sheet

Hello,

I want to make a file in infopath where we capture the service complains and generate the case id for different customer, every customer having a unique machine serial no and when customer giving m\c no and giving complete we log the request and generate the case id and save .after that if we put that log case id all the customer details appears. And this file we can log the many calls in signal file.
