Unable to read, and format microSD card.

Hi, I just bought a new 4 GB microSD card. I inserted it in a card reader to read it on my Windows PC. Instead of the common autorun window coming up, it asks me to format the drive. I clicked format. It took a while, and when it reached 100%, it says that the format was unsuccessful.

I also tried formatting it on the Disk Management. The results were the same.

I tried running "diskpart" in cmd.exe, and tried to "clean" my SD card. It says

"Diskpart has encountered an error: The request could not be performed because of an I/O device error. See the System Event Log for more Information."

I am also receiving this error. But one plus point is my memory card is working. I can able to read data and but can't able to format.

If anyone have solution for this, Kindly answer/give Comment to the below link
