How to create an editable PDF form from word document

Respected Sir,

I am doing job is DATA entry label. Some time I need to use same format to fill up the DATA and make a print on that as a HARD COPY to the jobber. So I need to make a editable PDF Form by which I can fill up the form and make a print from that editable form to serve my purpose.
