Someone changed my password and my phone number

Hi recently I found out that someone changed my password and my phone number and the city I live in on my email accounts. I was able to change my password and phone number, the city I live in is still not right however. I am now getting messages on my cell phone asking for the code so she can get into her email, and I got a message on Facebook, I blocked her. I have her phone numbers, how do I prevent her from contacting me or hacking my account again.
