VB 5

Hello,
how can make a simple form includes 4 spaces (These spaces enteres by the user) like:
1.Employee name.
2.Salary.
3.Tax.
4.total Salary.

for example i had entered employee name (John), and his salary (1000$), and his tax (2%)

I want in the end A massage box showing to tell me the following:
John has 1000$ salary and has 2% tax so his total Salary is 200.

I want this program in VB5 please.
