Thank you for getting back to me but I' m not sure if there is a glitch on my end or what but I am unable to input information in the fields it seems to be suggesting. I can only write text in the "Contact Us" box. Is this correct, if so how do I upload an icon photo for our software and a screenshot?

Yes, the information required must be entered in the Contact Us form. To add the icon and screenshot, put links from your website that point to them.
