Concanating text using vlookup forumula

Anybody help me to suggest a formula .

I am looking for formula at B11 .. it gives the below suggested text. red colored text will come from Main database sheet. Make sure the formula checks that Pin end finish & Box finish cell values based on I1 cell. the formula left the empty reference values and give text reference cells header name followed by text cell names ..

Sample result:

M002/24-5(I1) : Surface treatment as per Spec 1 ( Box end finish - Mn Phosphate)

M002/24-6 (I1): Surface treatment as per Spec 1 ( Pin end finish - Zn Phosphate)
