Unlocking a Samsung phone

I purchased a Verizon Samsung s4 phone then bought an unlock number with instructions. The instructions are not very good. I am supposed to use another sim card which means what, getting another phone? I cut one I had to the same size & it should work fine except it is also a T-Mobile & I don't know if it makes a difference. Also the numbers I received are : NETWORK = 00000000 PROVIDER = 00000000 SUBPROVIDER = 00000000 DEFREEZE = 00000000 EXTRA = 00000000. Which of these numbers do I use or do I use all of them. If someone has detailed instructions I would appreciate your input.

We would like to help you, but you must be able prove that you are the phone's legitimate owner and you can't. Everyday mobiles are stolen, although you may be honest, we do not wish to help potential criminals.
