help me reactivate my facebook account Closed

Hello, 
My Hotmail/Facebook account got hacked into last week on Wednesday at 1:41 am, my username that I use for Facebook was deactivated a long time ago, how do I reactivate my Facebook account, I have tried everything from filling in the form for registered email addresses I have send previous emails to. Could someone help me reactivate my facebook account thanks
