GTA4 has some VERY demanding recommended specs
Unless you have a quad core or newer and the best video card around its going to run like crap... sorry
same story for me, It runs, but at about 15 frames/sec

Core 2 Duo 1.8 overclocked to 2.5
4Gb RAM
ATI 4870 HD
Unless you have an i7 or quad core with lots of RAM and a spectacular video card(or SLI\crossfire), its not going to run well. You can consider upgrading your video card. If you have one of those micro PCs or a laptop, you cant do anything.
