Where do I enter the Facebook security code?

Hello,
I sent my ID and information and after two months I received a text with a code. Where do i enter the code into Facebook? All I get with I try to log in is the same ID information form. No place to enter the code.

Hi,

Clear your browser's cache and cookies and try accessing your account again. If you still don't have a field to enter the confirmation code you received, fill out this form to report your login issue to Facebook.

I was able to get a code, which I entered. I was told to change my password, which I did. Tried to log in and it looped around to the "Please Verify Your Information" page. I've sent info and ID docs several times on this page, but it always loops back to the same. Shows I'm logged in, but no access.
