If it is an old one, it might have reached its EOL (end of life). This happens with sold state drives such as flash drives and memory cards.
To know whether your card is unusable, simply try to add any file to it. If there is an error returned (drive not accessible), then, this is most probably the cause. Such cards cannot be formatted.

Insert this memory card in any Nokia mobile and try to format there.
