Why don't SD card work

Hello, my probblem is i canot see or access my memory card in my phone and pc. But i have do currently working Caredreader but do not detected memory card such as Computer,until iam do open a CMD to open a DISKPART next window open they will type LIST DISK then (also disk0) disk1 is shows
status is -no media. so i can't format or access memory card.
So will u please any person give me suggestions for my memory card (sandisk ultra)
