Referencing multiple Cells to one cell

Hello,

I have a table of values that i have screenshott here, for you: http://tinypic.com/images/goodbye.jpg

I also have what resembles a query form, which i have screenshott for you, again: http://tinypic.com/images/goodbye.jpg

The person would simply alter the values under name / size / color on the query form, which would generate the "Product ID" (AAA in the screenshot), which would then generate the availability of the product (in the form of in or out of stock) as well as the price of said product.

So for example, if i was to edit:

Name / Size / Color
A / B / A

Then the Product ID would be automatically generated to ABA. And the availability of product ABA and it's cost would be generated.

or, if i was to;

Name / Size / Color
A / C / B

Then the Product ID would be automatically generated to ACB. And the availability of product ACB and it's cost would be generated.
