Hi,
Select Get Started and follow the instructions to unlock your account. If you don't have access to your contact info, try these steps to recover it. Here's also a guide produced by CCM explaining how to recover a disabled/locked Facebook account.

Hi,
Because your account is not disabled, that's why the form to make appeal couldn't be sent. I recommend that you try the steps suggested by Facebook (to secure your account), even if you don't think it was hacked. Perhaps, Facebook has detected an activity that it doesn't usually recognize.
If the recovery process fails, don't make any attempt to access your account for 96 hours and clear your browser's cache and cookies. Then, after 4 days, you can try accessing your account again. In case it is still locked, follow these instructions to recover your Facebook account. You'll also find the steps to confirm your identity if you still can't access your account.
Note: To avoid duplicate threads, I've deleted the other one.
