How do i lookup a number value?

Hello,

I have the following problem. I have a column with a list of diagnosis (A) and coding numbers for each diagnosis (B). I have other columns with names of patients (C), their diagnosis (D). I would like for excel to automatically input the coding number in column E for the diagnosis I have entered in column D.

I would appreciate your help.
