Goto Insert Tab on Ribbon,
go to Quick parts (in the text Group),
select the field option in the drop down menu.
In the categories dropdown, choose Document information.
Select filename from the field names list,
Select one of the options from the format list.
tick the 'add path to filename box.
ensure the 'preserve formatting during updates' is selected if the document is likely to be saved in a different location at any time.
Select ok.
This will add the information to your document. Just cut and paste into your footer/header depending on where you wish it to appear.
and bobs your uncle, this will print the details as requested every time a copy is printed.
