Transferring survey results to table & graph

Hello,

I need to transfer my survey results to a table in excel and then to a graph, but as i have a few headings it doesnt work with a graph... Please can you help...

So I need to track by Month

THEN

Tiles:
Satisfaction Level
Response to request
Willingness to assist you
Level of Service received
Technical ability
Equipment recvd in good condition
Product Quality
Customer Service
Turnaround time
Collection time

Then by: avarage, good, vey good, and excellent
Then add: quanities for each

This is how i have it at the moment but its not right.......

First you have to transform you survey result in a tabular form

Could you please upload a sample file with sample data etc on some shared site like https://authentification.site , http://wikisend.com/ ,https://accounts.google.com/ServiceLogin?passive=1209600&continue=https://docs.google.com/&followup=https://docs.google.com/&emr=1 http://www.editgrid.com etc and post back here the link to allow better understanding of how it is now and how you foresee. Based on the sample book, could you re-explain your problem too
