Help with formula

Hello,

I am trying to create a fax coversheet that autofills in a specific cell the data that correlates to a particular name selected from a drop down. The workbook consists of two worksheets. One is the fax cover sheet (form,) and the other contains name, fax, and phone. I keep getting an #N/A error using VLOOKUP. Does anyone have any suggestions?
