Memory card not read by the phone

Hello,
my memory card is not read by the phone,when i put it in computer,the computer demands to format it,when i try to format the window fails to format,i have downloaded the format tool but also failed to format;please,help me to solve this.thanks.luhela george

i failed again to format my memory card with a format tool and using the control panel.help me again
