Damaged sd card

Hello,
I have formatted my sd card many times because my phone says it's damged. I've formatted it still my phone says it's damaged. What can I do?

Because it is "Really damaged" .

To make sure use it with your pc and see whether it is detected there..

How you format the card?

I just bought it a few ago, also I've tried formatting it using sd formatter still the same. My pc can also detect it but my phone can't.
