Hi Jb,
Are you talking about a hard drive or a game disk? I strongly recommend you to contact the manufacturer’s technical support center. I recommend you to restore the console to its default settings. Follow the link below for some additional suggestions:

If you cannot access the hard drive, there may be a problem with the drive. You may need to get it out of the console and connect it to a PC. Use the drive manufacturer’s tools to analyze the drive. If it is still inaccessible or cannot be formatted, unfortunately you may need to change the hard drive.
