The second table on the right is in G1 t o H5
in my excel version and according to my regional set up the dates in column H have to be filled as excel data that is m/d/yy and not as given by you. Note this change.
the fomula in C2 is
=VLOOKUP(B2,$G$2:$H$5,2,FALSE)+A2-1
