I have tried the option given to me to recover my forgotten password and secret questions and the proble is unsolve

The sign-up form is probably requesting the forgotten password to check what was the password.
Have you click on the enveloppe and scrolled down to write to Yahoo ?

I have not seen any envelop there. The form is only showing A. select a product B select category, c Sub-category. Non of these is reflecting to my Problem. If you can resend the form proper to me.
