All my icons on desktop and in start were changed into single

Hello,
Does any one can help me? I have met a problem, all my icons of the program listed in start menu and icons on desktop have change to single icon i.e, notepad ,while i asked documentation to be open in notepad. When i opened them, it asked me to save or run. I have know ideas to deal with this problem. Please help!! Thank you in advance.
