Windows was unable to complete (Windows 8)

Hello,
Recently I wanted to format my PC. I'm using a USB (16 GB) to format as I always do and it always works, now when I enter the USB it says: "You need to format the disk in drive F: before you can use it. Do you want to format it?"

I clicked Format Disk and then I clicked Start and it said: "Windows was unable to complete the format."

I tried to change it from FAT to FAT32 to NTFS to exFAT and none of them worked.
I unchecked Quick Format.

P.S: The USB is 16GB but it says at the capacity 64.0MB for some reason and the file system of the USB is RAW and I cant change it to NTFS.

My first thought would be you flash memory stick has failed and needs to be replaced.
