Help me repair my sd card help me

i buy a new camera and put a 4gb sd card into camera after a few days card is not opening and camera is showing format or full format
but format and full format not working then i connect sd card using card reader the computer is showing removable disk then i open it say format
i format it but it show "the removable disk was unable to format" then i goto computer/ management properties /disk managemnt / and select J (my removable disk) and format but not woking.... then i format to ntfs opsion tick and then format the disk management opsion was hang... then i unplug card reader then again plug card reader after the card is not showing it always showing pleaes insert a new disk blah blah....


other wise before ntfs format i use chkdisk opsion it says this disk is raw i forget was it say.... but someting in raw....


help what can i do...........
