Hi Niranjan,
Are you using an adaptor in order to connect the SD card with the PC? If you do, please try with another adaptor. Sometimes connectivity issues cause similar problems. Moreover, it would be helpful to test the card on another device or in another PC. If you have important data stored in it, try using some data recovery tools in order to save the information.
