Hi Nathan,
Sadly, it seems that there may be a problem with the drive. Please try to format it while you are using the PC in Safe Mode. If you are still unable to format the drive, I would recommend you to try formatting it on another PC. Furthermore, you may try using the Command Prompt. The commands should look like this:
Diskpart
List volume
Select volume n (n must be number of the drive)
Format fs=ntfs quick label=test (here you can change test with any label you like)
Exit
These commands should format the drive in NTFS file format.
