facebook disbaled my account once without any reason and asking for the documents i'll send them my ID and my account activated in 2 days.
My account name and ID name was same.. if fb did't activate your account it may be the reason of your Account name and ID with different name.

If facebook profile created with the name of business than you need to submit your company pan-card.
