Please my facebook account is hacked and how do I retrieve it back again and also prevent it from such action again? And I suspect is also with my phone too so what should I do please?

First of all visit the official Facebook hacked account website: https://www.facebook.com/help/1216349518398524] then click on the option "I think my account was hacked or someone is using it without my permission.
Then follow through.
