Windows unable to format memory card

When I insert my memory card into my computer it prompted as need to format. Tried to format it shows like "windows unable to format". Tried to format with disk management and diskpart and cleandisk and some thirdparty softwares and command prompt. but no use.
Can u tell me the perfect resolution for format the memory card. I lost my 4 GB and 8 GB memory card. I want it back.
HELP ME....!!!!!

I tried also in safe mode... but no use... :-( You have any solution......????

If its in warranty please replace it because memory is corrupted
