Hey there, eshwar.
If you've tried CMD and Disk Management already, I'd recommend that you take a look if the manufacturer offers a formatting tool for their USB flash drives and memory cards and try to format it this way, to see if this fixes the issue. Other than that, I'd suggest to try and format it by using a different computer, to see if the same thing happens.
