Index , array and need help with print

Hello,

I have a database and I created a form using this database . I used "index" syntax and it works but my problem is I need to print this form with the variable data and it is around 3000 records.
To clarify that I have to put the serial no. from 1 to 3000 and this cell related to the database and also related to the form.

May I ask someone to help with that?
