Cpu fan makes a lot of noise

Hello,
my cpu fan makes a lot of noise and it makes a weird sound-like when a door is being opened. it was repaired about a month ago and I think it was also cleaned at that time. moreover it takes a lot of time to shut down. any help, please?

It would appear that it needs to be repaired again.
