What methods have you tried?
Try reformatting the drive via a different computer/device, to see if the same thing happens. Try downloading the USB flash drive manufacturer's formatting tool (most manufacturers offer such tools for their pen drives and memory cards), to see if this helps you format the flash drive.
Unfortunately, if nothing helps, this type of behavior might mean that the flash drive has failed.
