Hi Chris,
Could you remove the card from your phone and try to connect it to a PC? You may need to use an adaptor. Search for the card in Disk Management. Try to format it. If it is not possible, run Command Prompt and type the following commands:
Diskpar
List disk
Select disk N (change N with the number of the drive)
Clean
Go back to Disk Management and try to format the drive.
