I m not able to formate memory card

Hello,

i m also not able to formate my m.card...actualy i simply copy & paste any thing in my phone,,it shows all done,,but when i check it in my phone there found nothing that i pasted...plz give me any solution ,, and when i creat any folder and give any name to that then the folder contain two files automaticly in in that....i m not able to get any thing what is going on...plz help..
