Image Resolution Problem

I'm experiencing an issue with photoshop cs3, no matter what size I specify in the image dialog box, when I save the image as a png or jpg, photoshop assigns the image with a ppi of 240 and different dimensions. However when you open the png or jpg, the dialog box still has the dimensions that you specified. The psd file saves at the proper dimensions. This is a problem if an online POD site requires an image at 12x16 @ 200 ppi. This just started doing this recently. Images that were saved at the proper dimensions a year ago now have a 240 ppi tag and cannot be used. I am no longer able to generate designs at the dimensions I need, 3 months ago this was not a issue. Any insight on this issue?

In mac you can change resolution by

Choose Apple menu > System Preferences, then click Displays and click Scaled, then select a resolution.

Ok, I just skimmed this, so bare with me....

When the file is first created, what are the formats? It is easier to go smaller with res than larger?!?!

Hit me if I am off base!

I had to do the same thing when I had to scale my bumper sticker, to a billboard recently! 200PPI is ok when it is is 120cm wide, but when it is the size of a truck, not so much!
