Hey there, s_uzair.
Try downloading the flash drive manufacturer's formatting tool, to see if you're able to properly reformat the pen drive with it. Other than that, I'd suggest that you try reformatting the USB flash drive with a different computer/device to see if the same thing happens.

i tried But Fails
now i connected my disk to windows 7 , windows does not shows size of usb disk its just shows Removable disk (f:)
i tried via disk properties then msg appears windows can not access the disk

If nothing works for you, unfortunately it might be a failed USB flash drive. If this is the case, I'd recommend that you contact the reseller you got it from or the manufacturer and ask if you could get it replaced, if it's still under warranty.
