Hi Raj,
Are you using a Windows OS? What file format do you select when you format the card? Open Disk Management and check if the format appears the same after reconnecting the SD card. Are you using the SD card with an adaptor? I strongly recommend you to check the card on another PC and if possible on another device, like a tablet or a smartphone. If you were using quick format, try using full format.
