Did you try to format that drive using HP-USB-disk-storage-format-tool?

Hi Kailash,
Could you tell me how are you trying to format the SD card? Are you using some kind of adaptor to connect the card to the PC? Is the card usable and fully accessible? Try formatting it from Disk Management, right clicking on it. If that option is not available there may be a problem with the card. Have you tried formatting it while using it in a device like a smart phone, camera, tablet, etc.?
Additionally you could try using Command Prompt. Type the commands:
Diskpart
List disk
Select disk n (n is the number of the drive)
Format
Format fs=ntfs label=(name) quick
Let me know if there is any progress!
