Cannot File Instagram Deactivated Form

Hello,

My IG was deactivated out of the sudden and I've been trying to fill out the form in the following link:

But once I hit "sent" the following error is returned:

"Your form could not be sent

Log into your Instagram account for more information about how to request a review."

This is quite frustrating as the reason I'm trying to appeal is to be able to log in. Once I try to log in directly "user not found" is returned.
