Usb not formatted and vlume lbel is missing.

Hello,
i am trying to format my usb but it is not formatted as it cannot format usb as volume label is not given.And sometimes it displays a message that windows are unable to complete the format.We I insert the usb its name that displays is removable disc.It is also not renamed.When tried to rename it displays a message that volume label is not given or invalid volume label.Kindly inform me how should I format my usb I really need it urgent.Thank you.
