Cannot format my 16 gb sd card

Hello,

i'm having problem with my 16 gb sd card
i am using it in my htc android phone
i,m having problem to format it
if i do format it through my phone all the data comes back saying"sdcard read only tapping it"
in windows it won't let me to format says "unable to complete format"
please help

Hi,

Try this tool to format it on windows:

https://ccm.net/downloads/tools/4639-hp-usb-disk-storage-format-tool/

Be sure to remove password from it as well as lock for write protection to be able to format it.

but it didn't work
i says"failed to format device"
no password is set and it is a micro sd card and there is no lock on it
