Hey there, Jigs.
Try those solutions with a different computer and try to reformat the USB flash drive again, to see if the same thing happens. You could also try the pen drive manufacturer's formatting tool, to see if you'd be able to reformat it this way.
Unfortunately if nothing seems to be working for you, it might mean that the flash drive has failed. It's not uncommon for a USB flash drive or a memory card to lock itself like this when it fail.
