Hi Hari,
Could you tell us if you were using the card with this phone or you have been using it on another device? I would recommend you to check the card on another phone or, if possible, on a PC. Go to Disk Management and check if the card appears with a drive letter and a file format. If the card is not recognized in another phone or on a PC, it may have failed and you may need to replace it.
