What can I do if my Instagram account has been disabled

Hello, my Instagram has been wrongly disabled for “pretending to be someone else”.
This account is important to me, I had an important photos and concert reports on it. I tried to appeal this decision by sending the form but unfortunately I get an error when entering my username
when I give my username, I get a message saying "The URL of the shortened link is not the username" so I can't send the form .
What can I do

Hello, I have in my archive an old story that was originally setted on "close friends".
Now I'd like to put this story in my story highlights, but in a public way, for everyone, without the restriction of "close friends"....
Is this possible?

I accidentally removed the link of my profile from my boyfriend's bio. Now, when he wants to mention me in his bio, the tag is not linked with my profile anymore. Is there any way to undo the function "remove link" ?

Please help. At all my posts on Instagram I cant see or reply to comments that are made at my posts. Tried everything and all the settings seem correct.

My account has been disabled for pretending to be someone else that someone Reported. But I used my account real Profile and Real Photo. Facebook made me wrong disabled. 

If you think i made a mistake, please let us know. I used my account over 6 years. I am using my facebook log in gmail is ***@***

I report my issues to facebook team over 25 times. But they reply me like that,

We can't help you with your request until we receive an ID or other document that we can use to confirm that you're the owner of this account. The document should include your name, photo and date of birth that matches the info on this profile....
