Two ways sort of vlookup, pls help!!!?

My question is to look up the name like Jack , Kate etc when the enter the formula. but the condition is that only if for example, col c 3 and col a date match it should show the name or else n/a. that means that only if the index time and date match the name shows...Please solve this for me which ever way you can thank you.
